At Optimum Rehabilitation Center, we recognize that patients have better things to do than sit around in a waiting room. To minimize time spent in our office, we encourage new patients to fill out Patient Intake Forms prior to their first visits. Additionally, we ask that you bring the following items to your first appointment:
- Driver's license or photo ID
- Insurance card
- List of all your current medications (with dosages and frequency)
- Patient Intake Forms
